I have used that mixer long time ago. If I remember right it is Tore Seem TSM6/2 or something like that. Full of NTP M100 opamps. PPM driver uses NTP log amp modules. JS transformers. Faders are 60mm carbon (that can be a problem). Switches are Elma. Well built (except the faders). Phantom batteries are impossible to get.

This is probably way too late, but I got my hands on a Kongsberg Våbenfabrikk FF 64 (serial no 1 - I guess this machine is kind of rare).
Anyway, here's the pinout. I tested it yesterday, and this should work:
8 input
9 input
10 +24V
11 output
12 V0
(I also connected pin 13 to chassis)

12 should also be connected to output
Got mine working. It sounds allright. Not as good as my NTP 182-120, but nice. Should work good on drums.
