How to convert balanced +4 dBu to guitar pedal input and back to +4 dBu
balanced out?

a simple resistive array should do it. A pad, with a hi impedance output. Say 10k output impedance and 20db attenuation. Wouldn´t be difficult calculating one of those. try searching for attenuators and pads...
 
Build yourself a 'reamp' type circuit (check the Jensen website for a good schematic), which will convert eh +4 balanced signal to 1/4" unbalanced for the pedal, then run the output of the pedal back through a DI into a mic preamp.

Not the sexiest "all-in-one-box" kind of thing, but it works great and allows for lots of user variables.
