I don't think you need a current source for Q5--simply connect the collector to the positive supply line and make R9 about 1k.
Make D5 a diode-connected TIS98 and set R6 = 10k.
What does C1 and C3?
You might increase C2 up to 10 nF for better phase margin.
If you need higher slew-rate and open loop gain, two-pole compensation may do the trick.
And please redraw the schematic a bit--took me five minutes to figure out the bias of Q5 and Q6...
[Edit:] Wait, it still doesn't make any sense--you must have mixed up emitter and base of Q2.
