Fabio's "Neve Out" (Class A Driver) Board Question

Well, maybe I'm just dense as granite, but I can't seem to locate the bill of materials for this little gem. I tried searching, but didn't locate it. Anyone have it handy or point me to it?

Well, there are a couple of questions I have about the schematic. The labeling for RV1 doesn't seem to line up with the artwork for the pot on the schematic, but it appears that it's 1K. Is that right?

What's the adjustment procedure for RV1?

I see the table that gives the value for RGx depending on the desired gain, but I can't find out where the "gain" and "rg" connect.
