Ok, here goes:

I'm a little confused on the wiring for my current project, the Royer MXL 2001 -> tube mic mod.

Here is the power supply schematic (figure 3):

Question 1:
The PCB has two pads for voltage. Should these both be receiving 24v? Or should one be receiving 0v and the other 24v?

I am using an Amveco 62062 toroidal. It's a 2x12v @ 1A. Wiring:

Primary
Yellow (0v)
Black (115v)

Red (0v)
Violet (115v)


Secondary:
Green
Red

Brown
Blue

Question 2:
I need to create an out put voltage on 24v. If I follow Mark Burnley's guide here: http://www.diyfactory.com/data/transformer_connections.gif

That leads me to believe my hook-up should be:
Primary:
Black and Violet to Live
Red and Yellow to Neutral

Secondary:
Red and Brown tied together off board.
Green and Blue separated to each of the pads

Not sure about the secondary. This is where I need a lesson.
