Any info on an API 302-t2 transformer

I'm looking for info on the API 302-t2 transformer, if anyone has ever heard of one. Looks to be an output transformer as its mounted on a UAC-1 card. That one is another mystery to me! The card has a Melcor opamp and appears to be an early product of API. Thanks for helping me out.
 
I saw these on ebay last week.. Did you end up with those by any chance?

AP302T2 Output (75 to 150/150 +30dBm) - open frame pcb mount.

Looks like that is the same one.
 
I'm still trying to figure out what is going on with these cards. Of coarse the components on each card are different. I was hoping to go for a 312 type preamp. Seems like it should be easy enough but is it possible while using this transformer? The transformer has 6 pcb mount pins all numbered. By the looks of it
1 is ?
2 is connected to COM
3 is ?
4 and 5 are connected
6?
Any ideas?

I just noticed the remnants of the jumper from the output cap to pin 1.
Does it matter if 3 or 6 is + or -?
Also any comments on turning this into a 312 circuit?
