1512 mixer design (more 1512 meets 1646)

I've been working on a simple mixer design for a couple months.  Actually the more I work on it the more complicated it becomes.
Basically it's a 1512 front end with a balanced insert section (a la the 'simple unbal to bal converters' thread) mated with 1646s for L+R outs and aux sends.
I've taken schematics from the various pieces and smashed them together. 
What I'm unsure about (and would appreciate a little feedback) is:
-the values for the fader and aux sends
-if I need to worry about pin 1 on the LF351 (for use as an offset servo)
-if this will work and/or makes any sense.

I'm sure there are other minor errors I haven't checked yet.
And sorry for my jumbled schematic - it's my first.

A couple other little notes - the L+R outs are intended to sum passively, with gain being made up by another mic pre.  The aux sends should work similarly but they won't be attenuated so much - that is, they will not be gained up before going to effects processors etc.  I don't have resistor values for the outs yet as I haven't decided how many channels to build.  The power decoupling (C14, 15, 18, 19) is based on what I see with the S800 - I'm hoping that'd be adequate.
