Symetrix 620 Headroom Issue

I am repairing a Symetrix 620 A/D converter. When the analog input gets above -12 dB I start getting digital distortion in any recording device I've tried. DAT, CD, and SD II. I've used the AES and SPIF outputs with the same result.

I'm new to troubleshooting. Could any of you give me some ideas about where to start looking?

Is the test source distorting at those levels?

No. When I use different converters I don't get distortion until after 0dB.

It is definately a "new" issue after several years of use.
