Does anyone have a Tech/Trouble report form in excel/word format that they wouldn't mind sharing?  I know it's a simple thing, but I also know that I want it to be as useful as possible and you guys have probably worked out the bugs in your studios.

I think we still have the excel templete for it at work. Been so long since we made it that the assistants/engineers just make copies off a master hard copy. I can remove our logo and send it to you if I find it. My 2 cents and I wish we did this at the studio is to have the form be carbon copy pages. You know where it's got 1 or more pages under the top page and as you write  on the top page it copies it to the other pages. I think that is best. The idea is you have 3 different color sheets. The top one and the second one get turned in to the techs when a fault/trouble arises.  The third one stays in a binder in the studio. As long as the third one is in there by itself then the problem has not been addressed. When the problem does get addressed, Fill out what was done on the 2 pages that were turned in. The top page gets filed away in the tech cabinet or a binder in the tech cabinet. The second sheet goes back in the binder in the room so that any of the engineers or assistants can see what action has been taken or is pending.
