Headphone amp - does this design look reasonable?

I basically just tacked on the debalancing input stage to existing amp in my Auditronics console. Does this look reasonable? Any glaring flaws? Suggestions for improvement?

5534 wants an input bias resistor from +in to ground. Preferably not larger than 1M

now what if i want to drive 2 sets of bones from a Y cord, do i need another part?

At the input XLR connector, pin 1 and C1,C2 should have short connections to the chassis but not to the circuit ground.
