Do you have a schematic for the circuit this will be employed in?

More often than not, an SPST switch should be sufficient, between the bias voltage for the front diaphragm, and the rear diaphragm ("open" for cardioid, ie. rear diaphragm disconnected, and "closed" for omni).

You can see the front diaphragm is hard-wired to the +60v bias voltage, while the rear diaphragm is connected to the "common" of that 3-position switch.

The "mini" criteria could be fulfilled by something like Apem TL36 series switches. The ones in your link are (at least by comparison) quite humongous. Oh, and non-"anonymous" switches usually have datasheets, which clearly indicate pin associations.
