I have fixed this with replacing a couple of surface mount transistors, and one of the op amps in the past. I used an oscilloscope probe connected to a battery powered guitar amp to trace where the noise was coming from. I can't recall where it was in the circuit (years ago). The lesson I learned was that it cost me more, in my time, to repair the problem, than to get it fixed/replaced by DD/Avid. Typically they turn around the A/D board for a couple hundred bucks, and the whole board is brought back to spec (repairing/upgrading any other issues).
