Seek power adaptor with +5, +15, -15

Does anyone have a lead for a power adaptor with +5, +15, and -15 volt outputs? It only needs to be 300ma per output, linear or switching, and most any size and shape would work. I've come up with nothing so far and need this for a video adaptor. I really don't want to build it from scratch and I don't want to build something up with an open frame supply. Anyone have any ideas? Thanks.

A minimum load of 10% is required on output one to insure proper regulation of remaining outputs, so min.500mA to burn by your video-application plus additional resistor/lightbulb or whatever.
