Software problem or audio problem?

I'm trying to test all my studio gears and solve all the old problems set aside...
But I Am in a very strange trouble ( I hope I ' m not understanding something).
If I record a track on silence I have a strange pulsing noise ,
It's always there
with input  signal and without
also if I record it as blank track...
The strange thing : if I'm in input monitoring mode I don't hear nothing, after in play mode the noise is there.
digi 192 - headphones and Pro tools Master
Rosetta 800 - external preamps directly connected
AD16x / DA16X  From Amek Console to Amek console.
Could be a problem with HD core card or it's an audio problem?

Recorded in protools only with 192 input 1 assigned and connected to external preamp ( I've tried without preamp with same results...
Gain pushed up 30db  to making listen...

Very mysterious. You have a background noise, was this present all the time?  Question is, why is is it alternating..
You should start over and disconnect every part that could cause a noise. Try Pro tools first with your headphones and then systematically connect components. That's the only advice i can give

Yes the noise is always there, the strange thing is that in Input monitoring mode the noise go away.
For this I think could be a software problem...

How is your exact routing through the mixer? What inputs did you activate when doing this recording?
 
This is definitely a software problem!
It's the sync inside drivers that is broken somehow.
Reinstall drivers and control panel software for your card.
 
Problem discovered!!!
The noise is only in 16 bit session independently of sample rate.
24 - 32 bit completly silent!!!
I've tried to connect the interface individually one at time but the problem is always the same only at 16 bit.
At this point I think it's a software bug or a defective core card.
could be?
I've also cleaned the pro tools preferences and install the upgrade to 10.3.10 but without luck... ???

Looks like a lot of zoom over a LSB noise, only two values being adopted. Totally makes sense it's not present on 24bit resolution, they would be 50dB lower which makes it invisible for that level of zoom, and once at 24 bits any signal that used to be analog will be dominated by the analog noise.

I guess the problem is too much zoom over the track or if you are able to hear it too much digital gain.
