Discrete Op Amp with low, or trimable DC offset?

DOAs can generally hit < 20mV without trimming by using precision dual transistors for the input LTP. That said good precision duals (or quads) are ICs themselves. The iconic LM394 (now obsolete) was the standard input LTP in Hardy 990 DOAs.

Pretty much any DOA can be trimmed for DC, but they may drift with time and temperature.
