This one is not balanced and not constant impedance. It's just a simple resistor divider.
just use one of these. You would ahve to use the resistor calculator to find the values to put in but this 4 pole 22 position switch made by alpha will work nicely for you. Plus if you ever need to go balanced you can...

Yep, we both meant the same, ehh, monitoring by eye, not the monitoring by ear. The audio-path between in & out is all passive, right ?
