Quiet PEQ

I just finished assembling my Classi PEQ from Don Audio. It's been a lot of work trying to figure out how to build it without instructions but I have it finished. It's only producing signals at low volumes when engaged and then no signal when bypassed. I'll attach the limited information that I was given in hopes that someone can find a flaw in the suggested components/connections. 

Suggested Resistors:
Filter:

R1 = 75R
R2 = blank
R3 = blank
R4 = blank
R5 = 10k
R6 = 10k
