I've searched a bit and can't seem to find anything on here that would be suitable.  I wanted to build something into my Igor monitor controller if possible.

You may want to check the Pro Audio Design forum. There are hundreds of posts about design of headphone amps; several of our members are also contributors there. Just do a search for headphone.

I haven't tried Wayne's Class A output board as a headphone amp. It would be my first choice.

For clients here I built one of theses http://www.jlmaudio.com/shop/headphone-amp-kit.html running +/- 24v with OPA2604

Plenty of headroom, and sounds good.
