Print the schematic of the opamp, measure the DC voltage of every node, write the numbers into the schematic, scan it and post it here. Without at least this level of information only the oracle of Delphi could help you.

Thank you Samuel, but I have dismounted the components from the boards and eventually I'll try again in the future ... although I have sent an email to JLM Audio asking if they sell only the PCB for this project
Do you think the simplified version of the circuit I have posted is working?

The pin numbers on the IC in the original schematic are correct, but perhaps the transistor pinouts you used are incorrect? Those are not listed on your schematic. It's so simple that I can't imagine anything else that could go wrong. Does the bias LED light up? Best of luck if you attempt this again.
