Doesn't look like any sort of rocket science...
Run-of-the-mill passive tube mic power supply. You'll want to scale those resistors depending on what transformer(s) you'll be using.
According to the schematic of the CV60 mic (part name for the amplifier body without the capsule?), you'll be needing 145v @ 1mA (rounding up), and 5.7v @ 300mA.
