Classic API - VPR-500 Building - problem- EQ module burned

Hi Everyone :)

i have a problem and i dont know how to find it ...
after finish building this project,  (the PSU and the VPR slot box)
ive checked the output voltage from the PSU cable,
the readings  was perfectly correct,

VPR 5 pin

pin2  GND
pin3  +16
pin4  -16
pin5  +48

I've started to month the 500 series modules,
first: CP5 pre amp, Lindel Audio EQ , and x2 DBX 560A compressors ,
ive turned the power and everything worked perfect!

2 weeks later  ive turned on the device again
ive notice that the Lindel Audio EQ power LED is not engaged

ive disconnected the modules  and mounted them again, to check that they siting properly,
turned the power,and still  the power LED of the EQ is off
after few minutes the transistor BD140 on the "GAR" start to "smoke" i guess is burned,

now i dismounted everything, and i need help...
what should i do now?

Who built the gar2520?
was the opamp tested before?
 
The gar was bought with the EQ
The thing is nothing is working right now, I'm afraid to connect the units before proper check, I need help how to check the PSU , and the sockets

Check what voltages are arriving at the 500 series connector

then take out the opamps, stick the module into the rack and measure voltages at the opamps pin sockets,  probably you will need aligator clips to do this
