The perfboard layout back on page 1 showed an (1N)5817 Schottky diode in series with the 9V supply coming in. It didn't look like there was 47R next to it on that particular layout. YMMV :)

Op-amps don't often spontaneously die
I tend to agree. But it may be hard to figure out what exactly happened after the fact. Especially since it seems to be working now with a good opamp.
ESD, voltage spike from a crappy wall wart?

I have measured the black diode near the DC input and this one reads OL both ways (I didn't take it off the board).
Assuming this means open circuit, then in case the polarity was reversed at one point (after the diode "died" for whatever reason) the opamp would see the full reverse polarity supply voltage, no?
