Unknown 6.35 Female Jack connector

Hi everybody,
I need to replace a female jack connector on a bass amp (the brand's name is "Epifani"), and the dimensions of the footprint are a bit unusual :

10mm between sleeve and tip, 16mm between switches

Otherwise it looks like any other connector :

I've contacted the manufacturer a week ago, no reply yet.
I think I've also seen the same connector on a Markbass amp lately.

Also I've browsed several suppliers catalog unsuccessfully (Mouser and others)

If anyone knows where I can get these, that would be a great help, thank you!
 
Is that hole centered (in the middle of that 16mm width), or is it offset to one side?
 
Just to say that I've blamed Epifani a bit quickly, my ticket has been lost for some reason and they finally answered and even called me.
The brand who produced these connectors has closed, so they say to put any connector with the pins facing upward to avoid touching the board, and to use wires.
 
Makes one wonder how much cheaper those jacks were, than anything already available in a way more standard footprint, that made them go that route...
