I'm not an expert, but trying to help.

Mic input transformer is connected to a switch for high impedance and low-impedance. So depends on using it as high (1k2ohm) or low (300ohm) input impedance. I suppose you're using carnhill vtb9045 transformers? If so:
- low impedance: connect pin 2 to 3, and 4 to 5
- high impedance: connect pin 4 to pin 3
in both cases pin 2 and 5 to xlr
also connect pin 8 and 9

Line input transformer:
- pin 9 to 10
- pin 7 to 8
- pin 3 to 4

Yes, with the carnhill VTB9072 for example.
