Identify and Replace old Rotary Switch

Hey folks,

Long time lurker, first time poster. I've been knee-deep in a microphone preamp project (racking a pair of Gates program amplifiers and building a power supply) for a while now, and it's almost finished. Just have to replace a few caps and wire it up! However, the original console only had one high-pass filter, and I'd like one on each channel.

They used these 2-deck rotary switches, with 12 poles and 4 total positions (I believe that terminology is correct, but I'm a bit of a newbie). Pictures included for reference. Where the hell can I find these? I'd like to rebuild the one I have as it is pretty worn out, so I'll need two. Thanks for the help!

Could you please slightly bent the capacitor (0,01uF) on the top of the switch so the center of the switch can be seen?
 
A 12 pole 4 position switch would have 60 solder terminals. Yours doesn't, so something has been lost in translation.  If you count the number of terminals and confirm the number of switch positions, I or someone here can probably make an educated guess about the actual configuration. A clear photo of the rear of the switch would be helpful. I probably have one available, whatever it is.
