Does anyone know what kind of circuit this would be?

Looks like a KM84-like circuit. Quite expensive for what it is, you'll need a specific body to fit that suitably / reliably, and it's likely going to be noisy as hell (no attention given to the high-impedance area of the circuitry).

In the comments someone mentioned the noise issue. Will be enough to use a teflon isolation for the gate connection or the closeness of the transformer also have something to do?

I guess it could be moved away from the transformer ?
